At the Northeast Gas Association, we, along with our members, believe that a critical component of continuously improving our industry’s safety performance is the development and sharing of “lessons learned” from events that have impacted the safety of the public, our customers, or our employees.

Behind every significant incident in our industry, there are real people - families, communities, and workers - whose lives have been forever changed.

As we continue to improve our safety management systems and drive toward the goal of zero incidents, it is our collective responsibility to continuously learn by engaging all employees in regular reviews of such tragic events.

It is in this spirit that we launched this Remembrance page to keep these costly and important lessons alive.

This page highlights significant gas-related incidents, including key findings and lessons learned. These events serve as solemn reminders of the critical importance of our work to ensure safety. By revisiting these events with respect and care, we will:

• Honor those affected by remembering the events and those impacted while redoubling our already strong commitment to safety.

• Learn from the incident investigations' results, evaluate them, define learnings, and act on them to strengthen our safety performance.

• Foster continuous improvement by ensuring employees understand these events and can use the lessons learned in their daily work.

Shared industry learning builds defense-in-depth. It reinforces the critical importance of fostering a positive safety culture as we move forward, building toward a future with zero pipeline safety incidents.

Attached you will find a link to these events that have been investigated by the National Transportation Safety Board, which will connect you to the investigation details, the final report, including recommendations, as well as a link to the docket and NTSB Board Hearing. While the learnings from the NTSB investigations provide recommendations for a variety of key stakeholders, the content on this site is directed at learnings and actions that gas operators can consider taking to improve safety performance.
